After Gensokyo is destroyed, Reimu sets out to investigate what happened and restore the barren land. Story Mode follows her journey through a sequence of stages, with a visual-novel narrative alongside the music gameplay.
In Kagura stages, hit notes as they travel through four or six lanes, aiming to build combos and score well. Danmaku stages combine rhythm play with bullet-hell dodging, and set missions to complete. Free Mode lets players choose songs for rhythm play outside the story.
Progress through the game to obtain Mitama cards and unlock content, including songs. The base game includes 68 songs, with additional song packs available. Players can adjust settings such as note speed, note design, and the placement of the combo display.
Reviewers often praise the Touhou music selection, including arrangements and original tracks, and enjoy playing them in the standard rhythm mode. Some also appreciate the four- and six-lane options, adjustable settings, and the opportunity to improve across different difficulties. Opinions on the combined bullet-hell stages and chart quality are more divided, with recurring complaints about managing both at once and inconsistent note patterns or timing. Song-pack costs and story-related unlocking or collection grinds are also common concerns.
