Set during a zombie apocalypse, the game follows four survivors crossing the Deep South, from Savannah toward New Orleans. In campaign chapters, players fight their way through infected areas and try to reach each safe room together.
Players use firearms and melee weapons against common infected and distinct Special Infected that can pin, drag, or scatter members of the team. Staying close, managing supplies, and rescuing teammates are central to making it through each chapter. Versus also lets players take control of Special Infected.
The AI Director changes encounters in response to the team's performance, adjusting elements such as enemy numbers, routes, weather, and effects. Campaigns can be played cooperatively, while Versus and Survival offer other modes; each campaign run can unfold differently.
Across the supplied reviews, players often enjoy the cooperative teamwork and the unpredictable encounters shaped by the AI Director, which help make repeat runs feel different. Reviews also frequently value playing with friends, community-made mods, and the range of melee weapons and infected. Some players describe Versus lobbies as hostile, particularly for newcomers.

| Multiplayer | Left 4 Dead 2 supports single-player with AI teammates and 4-player co-op campaigns online. It also supports online PvP, including Versus where survivors play against infected; reviews explicitly describe this as 4v4, implying up to 8 players in standard PvP. Official content includes Campaign, Versus, Survival, Scavenge, Realism variants, and Mutations. Local co-op exists, with reviews citing 2-player split-screen and split-screen via configuration; LAN play is also cited in reviews. |
| Multiplayer & mods | Steam Workshop, Source SDK, and custom servers extend multiplayer with custom campaigns, maps, mutations, skins, and server rules. Reviews also mention modded/community servers supporting higher player counts than standard, such as 8 survivors or 10v10, but these are not standard official limits. Some reviews note split-screen may require configuration. |
