Set in a comic-book-styled world of supernatural horror, the game follows a priest or journalist searching for answers amid an outbreak of madness. Each character has a distinct set of combat abilities and a separate development tree.
This is a fast-paced, old-school first-person shooter: fight through enemy-filled levels, collect keys to open routes, and search for hidden areas and supplies. Combat centers on movement, switching among guns, and using abilities against groups of varied enemies.
Combat experience and skill points let you improve weapons, character attributes, and abilities. Weapon upgrades can change how guns work, while a dynamic madness level affects both the audiovisual presentation and gameplay. New Game+ offers additional customization, and an endless mode pits you against waves of enemies.
Players who enjoy the game often highlight its hand-drawn comic-book horror look, brisk shooting, metal soundtrack, and weapon upgrades. Some find the Lovecraftian atmosphere and combat especially compelling, while others say the experience becomes repetitive or frustrating later on. Recurring complaints include confusing or padded levels, awkward platforming, ammunition pressure, and voice acting that some players found flat or grating.
