You play as Robert Hughes, a scientist suffering from a plague who washes ashore on a remote island. Search for a cure and try to reunite with the researchers at the island’s outpost, where something has gone wrong.
Explore forests, cliffs, camps, research sites, and ruins to find supplies, maps, notes, and clues. The island map starts blank: use landmarks and compass bearings to triangulate your position and chart its terrain yourself.
Study plants and fungi at research stations to discover their properties and synthesize medicines. Find water and shelter to manage fever, dehydration, and exhaustion, while using stealth and careful movement to avoid a creature that stalks the island.
Players who enjoy Miasmata often praise its hands-on triangulation mapping, island exploration, and isolated, atmospheric mood, which make discovery feel personal and tense. Some find the slow pace and momentum-heavy movement frustrating, and creature encounters also draw criticism.

Long ago, the island was inhabited by a group of indigenous people, whose extant monuments still decorate the island landscape. The island is now home to a scientific research outpost and it is your hope to reunite with your colleagues working there. Unfortunately, you'll soon discover that something has gone terribly wrong.
During your adventure, you will encounter a mysterious and deadly creature. This creature can stalk you for miles, lurking behind grasses and vegetative cover. By treading carefully and with stealth, you may be able to elude the creature. If you are careless, however, you will be forced to confront the creature head-on.
