In a post-apocalyptic open world, you lead a small community of survivors trying to establish a foothold. Choose who to recruit, where to settle, how to respond to other survivor groups and which community members to take on dangerous expeditions.
Explore towns to collect food, medicine, ammunition, fuel and building materials, while fighting or avoiding zombies. Bring supplies back to upgrade and defend your base; each survivor has distinct skills and traits that shape how they contribute. Blood Plague and Plague Hearts add threats that can make expeditions and fights more dangerous.
The campaign is a sandbox in which you can clear Plague Hearts, complete legacy objectives and move your community to other maps. Difficulty settings can be adjusted separately for combat, community and maps. Other modes include the Heartland narrative campaign and Daybreak, a siege-defense mode for up to four players; co-op also lets you visit another player's community and help there.
Players who praise the game often enjoy balancing scavenging, base development and survivor management, with characters' different skills and the risk of permanent loss giving expeditions extra weight. Some also value the emergent situations and replayability across different communities and difficulties. Recurring criticisms include the repetitive supply-run loop, limited co-op interactions and frustration with required Microsoft account sign-ins.
